SERVICES ﷯All Hearts Homecare is a trusted and accredited provider of high quality, cost effective home care services that will achieve positive outcomes in the health and well-being of clients and their families. Our services include, but are not limited to: Personal Care, Home Management, Companion, Respite, Children's Personal Care, Intellectual Disabilities and Related Disabilities, Head and Spinal Cord Injury, HIV/AIDS, and more. Activities of Daily Living (2 hour minimum) We offer experienced and certified personal care support to assist clients with non-medical daily living activities such as bathing, dressing, toileting, lifting, transferring, eating, exercising, and walking. We enable clients to live independently in their own homes by providing assistance with light housework, laundry, meal preparation, medication reminders, shopping, telephone use, companionship/respite, and escort/transportation. Companion Care (2 hour minimum) We offer companionship services - which include, but are not limited to: meal preparation, light housekeeping, transportation, reading/writing, making beds. Two-Companion Care (2 hour minimum - 24 hour care also available) We provide exclusive personal assistance for two (2) individuals at the same time in the same home for optimal care and convenience. Overnight Care (12 hour minimum per visit) We meet various client needs by offering personal care services during sleeping hours at 12-hour intervals, including all activities of daily living, assistance getting to bed, and overnight monitoring. Live-in Care (24 hour minimum per visit) Clients who are able to furnish room and board for caregivers can enjoy life to the fullest through home care assistance that includes all activities of daily living and overnight monitoring. Also available for two-companion care. Advocacy/Consulting We offer consulting services free of charge that help relieve the stress associated with the eligibility/application processes for Medicaid, Alzheimer's Care, grants, and other insurance benefits. As a client advocate, we conduct pre-assessments of each client's needs to determine which available resources are appropriate for them. We help those who need assistance communicating with other healthcare professionals, and give them the support they need to complete necessary paperwork. ﷯Secure Caregiver Matching Caregivers are thoroughly trained in CPR, first aid, workplace safety, infection control, lifting/transfer, and other care-giving skills. They are also bonded, insured, and appropriately screened through rigorous drug testing, background checks, and health assessments. In addition to regular home care duties, our caregivers serve as escorts for clients who need to be accompanied to doctors' visits or other activities. For additional security, we carefully screen and monitor the driving and insurance records of caregiver escorts who frequently transport clients to various appointments. Prior to a caregiver assignment, we introduce each potential caregiver to the client. This "interview process" helps determine the right match suitable for the client's needs, and is free of charge. Also, our staff and associate partnerships perform monthly interviews, both with our clients and caregivers, to ensure quality care. Friendly Re-assurance Services All Hearts Homecare takes pride in our clients' appreciation of the extra effort we take to ensure their safety and satisfaction with our services. Regular, "Friendly Reassurance" services are conducted monthly, or as needed. These services refer to making regular client contact, by telephone or home visitations to assure the well being and safety of each client. Our Nurse Supervisor conducts regular supervisory visits to ensure that our caregivers are performing their skills, duties, and responsibilities accurately. The Nurse Supervisor also assesses the client's condition during the visits to address any changes. For compliance purposes, the Nurse Supervisor notes the client's activities and progress during the visits, and also evaluates the caregivers - carefully documenting and reporting her findings, to ensure the highest level of satisfaction with our services.
